PROGRAM ON THE RISE TEAM LACROSSE CAMP - late application procedure below
CHECK IN INFORMATION
Please bring to check in:
1. MEDICAL FORM & Photo copy of Insurance Card (found on www.friars.com on lacrosse page - click on camp info icon)
2. BALANCE DUE (make checks payable to Friar Lacrosse Camps
3. CAMP BANK DEPOSIT (please note deposits not be returned until check out - any camper who will depart early will be sent the remaining balance in the mail)
What do I bring to camp?
We suggest you bring the following items:
Bedding including pillow (for single bed), a fan, towels, shower supplies (soap, shampoo, tooth brush and paste), lacrosse gear including mouth piece, swim wear, footing for turf, grass and indoor gym.
We suggest that you do not bring:
Televisions (movies shown in the dorm lounge at night and between sessions), game boy or other electronic games, ipods ect...
The following are prohibited in the dorms:
Refrigerators and air conditioning units and other large electronic appliances.
Dorm rooms will be locked via code locks so campers will not need keys
When do I check in and check out:
Check in for camp will be START AT 2pm - Check out will be at 12 noon
Commuting campers (not staying in dorms) will be dropped off at 7:45am and picked up is at 8:00pm at Guzman Hall on days other than check in and check out
Where:
Drop off and pick up will take place at Guzman Hall (intersection of Huxley Ave. and Eaton St. type into www.mapquest.com for directions or go to www.providence.edu )

AGREEMENT OF RULES AND REGULATIONS:
Please note that Friar Lacrosse Camps, LLC has put in to place a list of rules and regulations for the safety of our campers and to maximize the opportunity for them to get the most out of the camp experience. All campers are subject to the discretion of the camp director in terms of their compliance with camp rules. Although it is our goal to eliminate any possibility of misbehavior, the director does hold the right to send a camper home, without refund, should the camper's behavior in any way diminish the opportunity for other campers to enjoy the camp experience, or endanger themselves or others in any way.
LATE APPLICATIONS:
Please do NOT mail any items to us after July 8, 2005. Simply fax a copy of your application to us at 401-865-1231 and bring the application with full payment on July 18th (along with forms listed above)
